Second Chance Cat Adoption Application
 
Our mission is to carefully match every animal with the best home for its needs and to ensure a long lifetime of safety, love, and companionship. There are many applications that are declined for a variety of reasons based on Second Chance's many years of experience with cat and dog adoptions. This does NOT mean that any volunteer of Second Chance is making a judgement about an applicant's ability to provide a good home for a cat or dog.
   
 
Second Chance typically limits the adoption of cats to within 60 miles of Raleigh, North Carolina.  
Adoptions must be completed in person and we do not provide transportation for out of state adoptions.

ADOPTION PROCEDURES:

At the adoption appointment, the medical background of the cat will be reviewed with you. It is important to bring a cat carrier to transport your new cat. If you do not have a cat carrier, the cat will not be discharged to you at that time. We request that you pay the adoption fee by check. If you need to pay the adoption fee in cash, the discharge volunteer will ask you for a form of picture identification. Your cooperation in this matter will be greatly appreciated. The applicant understands that any false, incomplete or misleading statements on this application is grounds for denial of the application and/or the immediate return of the cat to 2nd Chance. You will also be asked to review and sign an Adoption Contract at the time of discharge.
